You must have both documents completed & signed when you come for registration.

2009 Camper Health Form 1: To be completed & signed by Parent/Guardian

2009 Camper Health Form 2: To be completed & signed by your family Physician

PLEASE NOTE: ALL CAMPER HEALTH HISTORY FORMS MUST BE SIGNED BY LICENSED MEDICAL PERSONNEL (YOUR FAMILY PHYSICIAN).
(ACA accreditation and liability requirements specify exams must be within 24 months of camp attendance.
A new exam is not required for camp attendance if your child has had a physical/exam in the last 24 months,
BUT... a new form must still be signed every year by your family physician and we must have their current shot records.)

Week Long Camp Sessions
Registration: 4:00pm on the Sunday of your registered camp session.
Camp ends: 5:00pm on the Friday of your registered camp session.
All campers must be picked up by 6:00 PM on Friday for week long camps.
We also invite Campers & their Families to join us for an optional supper, by reservation, at 5pm on Friday for $5 per person. You may make your Friday supper reservations and payment during registration on Sunday.

Our famous made-from-scratch cinnamon rolls will also be available for you to order during all camp registration times.
$5 per dozen. They will be ready for you to pick up when you pick up your child on the last day of their camp session.

You and Me Camp 1
Registration: 3:00pm, Friday June 19
Camp Ends after lunch, June 21

You and Me Camp 2
Registration: 3:00pm on Saturday, June 27
Camps ends after lunch on June 29

Twilight Camp
Registration: 5:00pm on Monday, June 29
Camp ends with lunch on July 1
All campers must picked up no later than 1:00pm
